What happened to George?

I am not really into errors so I don't know much about the topic but I'd like to think I can differentiate it from basic PMD.  That said, I have no idea. Seems like a bunch of extra metal in the king's head and possibly in the background is a diagonal 1 similar to the one from the obverse but that's just a possible explanation-I don't know.  Seems the king was punched on top of it.
